give permission for his/her participation in the Senior Religion Service
Practicum.
I understand that one class bell out of every five day rotation from
mid-September through April my son/daughter will be
at the agency assisting with their efforts.
I have reviewed the site they are attending and the other students they
are working with (linked above).
I understand that they will be driving, or riding with student drivers, to
and from the site.
[ ] Yes, my child may drive themselves down to the service site but I
ask that he/she not take any passengers.
[ ] Yes, my child may drive themselves and other students down to the
service site.
[ ] Yes, my child may ride with another student to the service site
but I ask that he/she not drive.
I further understand that my son/daughter will be under the direct
supervision of the agency with which they are volunteering.
