How to Apply - New Scholarship Applications:
Families are required to create an Ohio ID account with the state of Ohio. Visit the Ohio EdChoice Scholarship Program website for instructions on how to apply for an EdChoice Scholarship.
Once the Ohio ID account is created, please email the following materials to edchoice@summitcds.org for processing:
- Completed 2025-26 EdChoice application
- Required proof of residency (A utility bill less than 90 days old and showing both the service and home address as the same is the best option)
- Student birth certificate
- Authorization form to allow Summit to process checks from the state of Ohio
The required materials must be scanned and attached as separate PDF files. Summit will enter scholarship applications for students through the secure online application system.
New applicants to the EdChoice Expansion Scholarship must complete the state's income verification process to receive a scholarship award. The student application must first be submitted by Summit before income verification can be completed. Income verification information can be found on the EdChoice Expansion Scholarship Resource website, including a Helpful Tips document. Families may also forego the income verification process via a brief affidavit and agree to accept the minimum EdChoice Scholarship value ($616.60 for K-8 and $840.80 for 9-12).
To find Summit Country Day as a provider school, click “Open Search” under the Provider Search Page section. In the new search box, type “Summit” into the Provider Name field and select “EdChoice FY 26” for the 2025-26 Application Period.
Adjustments to a family's billing account will only be made after The Summit Country Day School receives official award notification from the state of Ohio.
